Abstract

Introduction

Lymphedema is a progressive disease that leads to chronic swelling. Lymphedema treatment includes complete decongestive therapy (CDT) provided by a certified lymphedema therapist (CLT). Locating CLTs can be difficult for patients and providers. Our aims were to 1) describe a method for assessing accessibility of lymphedema treatment in a specific region, and 2) apply this method to a specific region (Minnesota) to describe the lymphedema care in that region.

Methods

This cross-sectional study searched therapist directories on websites related to lymphedema and lymphatic diseases (e.g. lymphedema training programs, professional associations) to assess the current distribution of CLTs in Minnesota. Duplicates, therapists outside of Minnesota, or therapists no longer practicing were excluded. Data was geocoded, CLT per population ratios were calculated, and drive times were generated from each location.

Results

Five of the 15 lymphedema websites had CLT directories available, listing 428 CLTs in Minnesota. The highest CLT per population ratios were in regions with larger cities. Drive time to the nearest CLT was variable throughout the state, with the metro area having more CLTs within 15- or 30-minute driving distance. Northeast Minnesota had the fewest number of CLTs.

Discussion

Our methods reflect the difficulty a patient may experience while searching for lymphedema care. Rural areas have poorer access to CLTs both in absolute number of CLTs and driving distance to qualified therapists.

Conclusion

Access to lymphedema treatment varies greatly across Minnesota. Further research and targeted initiatives are essential to expand access to CLTs in rural areas, ensuring equitable availability of specialized care.
